CHICAGO—Jason Christopher Hernandez, 33, died May 13, 2011.

A funeral service with military honors will be at 11 a.m. Friday at Parr Funeral Home in Suffolk. Burial will be in Albert G. Horton Jr. Memorial Veteran Cemetery.

Visitation will be 7 to 8 p.m. Thursday at the funeral home.

Mr. Hernandez was a U.S. Navy veteran and worked as an electrician.

Survivors include his mother; wife, Michelle Martin Hernandez of Windsor; daughter, Alicia Nicole Hernandez of Windsor; sisters Sandy and Tracy Hernandez; and brothers Terry and Frank Hernandez.
